Lion Air crew castigated in Bali crash final report (with photos)
So basically the pilots screwed up big time. Forget about the visual minimums, the autopilot warnings about terrain, and the fact that they couldn't see anything in front of the plane. Just go ahead, take manual controls, and fly the plane nice and hard into the ground. Or water. (airnation.net) More...

This flight crew, in particular the Captain, should be prosecuted and serve time for reckless endangerment and be banned for life from ever piloting a commercial flight again. This tragic accident was completely preventable. The airplane was performing normally including warning these numb nuts it was time to take it around. The crew was little more than chimpanzees on an excursion ride in a 737. It would be unbelievable if it weren't true.

The paint was not fully dry on the tail number and the cabin had that "new plane" smell.
